Waiver of Liability and Release

By signing this document, the parent or legal guardian confirms that he or she has authority to sign, has read the entire document, and has understanding that the document waives certain rights of the child(ren) and the person signing.

The parent or legal guardian acknowledges and agrees that neither City Lights Together, Inc., nor any of its officers, board members, directors, employees, agents, Instructors, or any other representatives (collectively referred to hereinafter as the "Representatives") can be responsible for any injuries to persons or property arising out of participation of the child(ren) in the City Lights Music Together Program. In consideration of the benefits to be derived by the child(ren) and the undersigned from participation by the child(ren) and any other person who may have any rights to assert any claim on behalf of himself or herself or the child(ren) with respect to injuries to the person or property of the child(ren), hereby waives and releases any claims for damages against City Lights Together, Inc., and/or its Representatives arising out of injury or damage to the person or property of the child(ren) and hereby releases and forever discharges City Lights Together, Inc., and its Representatives from all actions, causes of action, suits, damages, judgments, claims and guardians, representatives, heirs, executors, administrators, successors or assigns may have against City Lights Together, Inc., and/or its Representatives arising out of participation by the child(ren) in programs facilitated by City Lights Together, Inc.

• Class availability is subject to sufficient enrollment

• Any cancelled classes will be rescheduled
• Class schedule is subject to change
